Brooklyn Bears Nomination Form

Nomination forms must be received by July 14, 2017 for the induction year. If your nomination is not chosen for this year, you are able to re-nominate your candidate. The Brooklyn Bears Hall of Fame Committee will review all nominations. Inductees will be notified verbally and in writing.

Please provide as much information as possible. The selection committee will do additional research on the nominee if needed. Please, no self-nominations.
